The situation

Client is extremely suicidal but denies imminent status. Has a plan but won’t divulge. Client is threatening to end therapy because “doesn’t see the point”. Does the clinician give in to the client’s wishes and end therapy?

  1. Definitely predicated on the strength of the relationship between client and therapist. If I felt a strong relationship I would challenge this perception a little bit more. If this were a new client to me I would respect space, offer some alternatives (peer supports, groups, etc..) and educate about what I am seeing that concerns me. Other risk factors might also come into play (gun access, history of suicide, recent losses/stressors, other changes in mental status, physical health etc.. ). Sight unseen it would hard for me to determine of hospitalization is needed. Indeed a very complex issue for both client and therapist. Sean
