Going down in Flames

Going down in Flames

Feeling depressed today, more than usual. Seems like as each day passes, I get more and more into the abyss. I have been out of the house three days this week. I am happy about that. Just wish my mood could reflect it. I went to my father’s today for my weekly visit. I am thinking of getting him a monthly pill box so I don’t have to go over every week. We’ll see. Today he asked me how long he has to take his meds. I told him forever. You would have thought I just gave him a death sentence. Idiot.

I got good news today. My psychiatrist is back in the office! I haven’t seen her since September. I am trying to set up an appointment with her but she, of course, hasn’t responded to my email yet. I hope she responds soon. I can’t wait to see her. It’s been too long.

I have had a sleepy day today. Went to bed with pain and woke up in pain. Pain meds caused me to go back to sleep. Even now, I am having a hard time bearing weight on my left foot. It started on the way home from my father’s. There has been some temperature degree changes in the last 24 hours so I am hoping that is the reason. I really can’t bear to be in physical pain while I am suffering from psychache. It is just too intolerable.

I haven’t finished typing up my short story. I really wanted to work on that today but haven’t found the time to do it. I might type up one page when I finish this blog. Depends on how I feel and what motivation I have left. Just writing this is difficult. I am being distracted by Twitter and my damn phone keeps going off with text messages. Most of these text messages are from the T, saying that buses are being delayed due to traffic. I get them every day around this time.

writing and dying

I typed up a few pages of the short story I am working on. It’s basically an autobiographical story of my struggle with psychosis. I still have two pages to type but can’t find the motivation to do it. I keep staring at the notepad but nothing is coming. I am just staring at the words, wondering if they are the right ones and am doubting myself.

My therapist had some suggestions on trying to get me back to better sleep health. Unfortunately, I didn’t do it today. I wasn’t tired come three o’clock but got really zonked around five. I still am fighting the urge to just go under the covers and take a rest. Only problem with that is that I will be seriously disrupting my sleep cycle, again.

I told her I am having trouble taking my meds. I don’t want to take them because I don’t think they are effective anymore. I am just so depressed, I just can’t see the point. I also told her I really want to be writing my suicide notes and letters. She almost started flipping out. I never heard her so restrained before. We were actually “talking” about this stuff rather than dusting it under the rug, like it didn’t exist. I really liked talking about this stuff. It gave me some perspective about the pros and cons of actually doing it. We didn’t discuss that, but I was thinking about it as I talked about the need to write. I told her I had written a goodbye letter to Jobes. It was basically saying yes your work helped me but it didn’t save me. I never sent the email/letter. I told her that I can’t be saved. It is my destiny to kill myself, I am very convinced of this. I don’t know why, maybe it’s because I think about it so much that I don’t think I have any kind of future or anything. The weird part, is that I don’t feel hopeless. She did the cubic model of suicide on me. I was kind of shocked that she remembered. I am no where near suicidal range at this time. I am not actively planning my death, nor have I picked a date. I just want to self-combust so I won’t have to. But I don’t think that is likely.

I just feel really depressed. My therapist also read my blog that I wrote last night. She said that it was very coherent and eloquent. It is funny because it took me several times to get back to writing it. I would start writing and then find the need to check Twitter or Facebook. It took me almost an hour to write. And I was mostly writing how fucking sucky I was feeling, which was hard because I really couldn’t put to words what I was feeling. It was a very difficult blog to write.

Foot/ankle are behaving for now. Last night I was in misery. It literally felt like a battle between the two as to which part was going to hurt more and then my big toe settled it by buzzing up a storm. I hate when my toe buzzes. It is so unsettling. I don’t remember if I took pain meds or not. I really wanted to OD on Neurontin. It was that bad. But I didn’t take any because after an hour, my toe settled down. Foot and ankle were still at it but not in the same intensity. I have been using my pain meds sparingly. I don’t know why this is. I guess I want to feel something and my ankle/foot is giving me something to feel other than depression. Maybe it is a form of self-punishment, I don’t know. I do know that the pain is tolerable to my warped mind. It gives me something to focus on rather than my stupid, stinking depression. I spoke too soon. Just moved my ankle and it has exploded. Just like that it can become a 10, easy. Now I am really going to get sleepy with my pain meds on board. I don’t think I am going to make it to eight. I guess I will be taking my night meds early and calling it a night.

My father called saying that his meds are almost out. I told him I will come over sometime tomorrow. I don’t mind because it gets me out of the house. I rather do it tomorrow than Saturday. I like to have the weekend to do nothing. I am trying to go out during the week, like I did today, so that I can rest on the weekend. I don’t know how likely that is going to happen, but we’ll see.

Three feet from the bureau

Three feet from the bureau

In October of 2012, I was caught in another flare up. It was night time, well past business hours of doctors and therapists. Normally I am good at reaching out. I text my therapist. I write in my journal or blog to get through the episode. But this night was different. I snapped. I couldn’t bear any weight on my foot that night and it really made me feel like I couldn’t go on. I was so tired of feeling like an invalid. I wanted to die in an awful way. Problem was that I couldn’t get to my bureau three feet away to get the meds I needed to do me in. Any time I tried to move, I was in agony with horrible foot pain. I could barely move my foot out of the covers of my bed let alone stand long enough to shuffle to the bureau. I cried as there was nothing I could do but take what I had at my bedside. It wasn’t much but it was enough to make me unconscious and away to dreamland I went.

When I awoke the next morning, I was in a dreadful mood. I wondered if I had really done what I thought I did. I checked my pill box and it was empty. I checked the contents of my pain meds and it was close to empty. I couldn’t believe what I had done. But I survived it and am living to tell about it. How I woke up I don’t know. I felt ashamed of myself, not in surviving but of making an attempt and not seeking help. I had promised my doctors that if I felt like I did that night, I would call them. But no phone calls were made. I had a confidential suicide hotline to call but I didn’t. I had made suicide prevention part of my treatment plan but yet the crisis response plan went out the window that night. I didn’t implement any of it. I just took pill after pill until I passed out.

This scared me. I was no longer in pain yet who is to say that if this happened again I wouldn’t try to end my life again? Dealing with chronic pain is a beast. And so many times it doesn’t get a flag for suicide risk assessment. My better judgment wasn’t in gear that night. I not only wanted to end the pain, I wanted to end everything. I spend the next few days in a haze, and not from medication. I was scared to let my practitioners know what I had done. I finally broke down and told my therapist. It was very difficult admitting my attempt. I then told my psychiatrist and she shocked me by saying that if I wasn’t in pain, I wouldn’t have done what I did. And it’s true. Chronic pain changes you, not only physically but mentally as well. It took me a while to write about what I had done on my blog. After all three feet was the only thing stopping me from ending my life or attempting to. I’m still fearful about making another attempt while in the throws of another flare up. I keep a small portion of my meds by my bedside so they are not lethal. I was lucky I didn’t need medical intervention the next day. I still am not quite sure how many pills I took that night. And that also scares me because who is to say that the next time I won’t count them out.

Chronic pain is not something to underestimate. Hundreds try to end their life year after year because of physical pain. Most people see their doctors before an attempt is made. The question remains whether an assessment is made for either depression or suicidal thinking. My primary tries to assess my mental health but mostly just asks when my next appointment with my psychiatrist will be. It might be followed up by will I call him if I feel like taking an overdose of my pain medication. Yet he knows he is not the first person I will call. In my order of people to call: my psychiatrist, therapist, primary, then ER if I can’t get a hold of one of them.

That night, I didn’t call anyone. All of my safety plans went out of the window. All because I was overwhelmed by excruciating physical pain. I think if I could go back in time, I would have taken the time to breathe. And think more of self-preservation rather than self-destruction. I am more aware now of what to do but it’s not that easy when you are in the heat of the moment. A month ago I was again in excruciating pain for three days yet suicide was the furthest thing from my mind. I knew what to do to cope with the physical pain. And luckily that didn’t involve a bottle of pills. I felt the attack come on so treated my pain much earlier than I did in October. This helped to keep the suicide demons at bay. I texted my therapist, I emailed my primary that I was in excruciating pain. I also got in touch with my psychiatrist who help me to see there was a tomorrow. Doing these things didn’t make my physical pain hurt less but made my psychological pain bearable. It helped me to cope through this rough patch.

Being mindful about pain is how I get through bad flare ups. I have no control over these pain attacks. But I do have control over what I do with it. I learned my lesson from that October night.

Sleepy and tired

Sleepy and tired

I didn’t do much yesterday other than go to my father’s to fill his medication box. I don’t know why he can’t do it but it is better that I do it so that I know he is taking his meds.

I have been so exhausted the last few days. After coming home and having dinner last night, I just passed out. I couldn’t keep my eyes open any longer. Today I plan on just sleeping until the Pats game starts. I think my sister is having people over but I am not sure. I thought I would write a blog now as I might not have a chance later.

The other night my mood was bad. I was fighting the demons and sleep, not a good combo. I wanted to read “Definition of Suicide” by Shneidman but all I did was thumb through the parts that I had highlighted and then tweeted them on Twitter. I then posted the ten commonalities of suicide, which was difficult. The last two, commonalities 9 and 10 were a tight fit because it almost exceeded the 140 character limit. I would post this book on my “useful book” page but it’s really got some words that maybe hard to follow on. I have read Shneidman’s works before so I am used to his vocabulary. Other’s might not understand his thinking.

I had tweeted that my suicidality was peaking and got a response from a fellow tweeter. He said that things will be better in the morning. I knew that was true but that didn’t help me now, in that moment. I felt awful and suicide was consistently on my mind. I wasn’t in imminent danger but I just could not stop thinking about ending my life and how bad my life was. The worst part of this, was that I could not express myself on Twitter like I can on my blog, but I couldn’t write on my blog either. I felt like words were strangling me, trying to come out but didn’t have an outlet. I felt trapped and isolated, like a bug on an agar plate. I had felt intense despair, hopelessness, anguish, and worthlessness all at once. It was terrible. And I couldn’t sleep because thoughts were keeping me up. I eventually took some Ativan and that calmed me down enough to sleep. But it was a terrible night.

I haven’t had breakfast yet and plan to soon as my mother leaves the house. I might make coffee today. Maybe that will wake me up enough so I don’t go back to sleep. I don’t understand why I am so tired. I slept good, even had a couple of weird dreams. I just took my morning meds and if it causes me to be more tired than I am, then I know the extra baclofen is the reason. The doc increased it when I saw him last.

Yesterday I did my exercises. I didn’t have too much pain so I think I am doing them right. I hope this continues. I see the PT again on Wed. It is going to be tight because I see her in the morning and then I have my therapist in the afternoon. I literally have an hour to get home or I am going to miss my next appt. I hope I make it home in time. I should as long as the trains and buses are running on time.

I can’t wait for the Pats game tonight. They are playing the Colts and I hate the Colts, especially their QB, Luck. I also hope the Packers win against the Seahawks.
