finding care in the off hours

Last night I was in the throws of pain again. But my thoughts didn’t immediately turn to darkness Like they normally do. I wrote an email to my pdoc about what has transpired during the week and that I haven’t been faithful in taking the increase in my mood stabilizer for reasons beyond my control. Mostly because I have been driving and I don’t want to be drowsy behind the wheel. I also asked her if there was a hotline number she knew of to call in times of distress. She gave me one.

This all lead me to thinking maybe I should have a blog about this important issue…finding care during the off hours. For most people in therapy, when they are in distress after hours their and before their next session is to seek help in the local ER. This can be costly, as copays have more than doubled to deter such visits. But for mental health, there should be an exception made as there is really no other place to go while in distress. I understand that the cost is higher because care is more urgent and is most likely is trying to ward off unnecessary visits. But when you are in crisis, how can that be unnecessary? For the mental health field, there are no urgent care centers to go to in distress. It’s either you see your therapist or go to the ER (Emergency room). There is no in between. SO what are you supposed to do when you are somewhat distressed and cannot wait till your next visit with your therapist?

There are self-soothing measures. You take a bath/shower, read, journal, brush your hair for 100 strokes, eat something, etc. But when all is said and done, and you still feel terrible, then what? Most therapists have some kind of plan in place, or should be able to give you a hotline number such as Lifeline 1800-273-8255 (US only) or the national hotline number 1800-784-2433 (US only). I have tried calling the Lifeline hotline but have never been patient enough to wait to get transferred to someone. The other number I have not tried. There is also a text # 20121 and you text 121help. I don’t know where I got that number from but when I tried it the other day, it didn’t work. I never got connected to anyone, but that might be because of the hour.

There are chat groups, I am told, where you can discuss suicidal feelings and not be “punished/banned” or turned away. Unfortunately, I do not have that URL to share as I have not looked into that chat room. But when you are in distress, are you really going to google something??

There is something called a Crisis Response Plan that I sometimes use when I am in distress. UNfortnately, the last few times I have been in distress, all of the self-help went out the window and I didn’t use anything. I just ruminated about what I was going to do. It’s not perfect trying to save your life when you feel like ending it. I was not in the frame of mind to seek help.

Therapists think that a suicidal person always calls for help when in crisis and that simply is not true. Most clients become impulsive, wants to get rid of the negative feelings NOW and are in my experience, not likely to reach for help.

For me, writing has helped but not everyone has that option to them. I will blog my distress and might be fortunate to have a few bloggers comment to show support or to chat. But that doesn’t happen all the time. Most of the it’s hit or miss. My frequent blog readers might not be online at the time of my post and so not get it.

Sunday I participated in a chat that was for crisis intervention and I learned that there was a crisis text chat available. The intervention was either through chat or text message. I am going to looking into using this the next time I am in distress or when my mood goes south. It’s not always easy to think of these thing when your mind is thinking of ending your life. To use a DBT term, you just cannot tolerate the distress because it is unbearable.

I have been trying to identify when I am in distress so that I can reach out for help but it’s not so easy. The last few time I have used distraction or music to help me out of the intolerable feelings. I also will write in my blog or my journal but it seems like all bets are off if I am in intolerable physical pain. Chronic pain mixed in with getting my menses and dealing with it has been difficult this past two weeks. The waiting for pain medication to kick in is sometimes not fast enough to deal with mentally.

I wish there was some help I could have regarding physical pain and lethality but there are few pain specialists that deal with mental health issues and fewer still, psychologists that deal with pain issues. Even crisis help lines don’t know hot to deal with physical pain that is behind suicidal ideation. In a perfect world, you would like to see someone that is well rounded in chronic health issues and suicidality. Unfortunately, I don’t think they exist or they may just be too far and in between to help the greater good.

When I was being evaluated at my local pain clinic, I saw a pain psychologist. His job was to help me deal with pain. But he didn’t offer me an real advise the first time meeting him. I would have to set up a series of sessions with him but unfortunately this happened when I lost my car and he is too far out to see. If I saw him with public transportation, it would be a two hour commute, both ways.

The hardest part of being alone with your thoughts is that you are left to your own devices before the next session or when office hours are available. I wish there could be urgent care centers that are specific to mental health issues. Because not all crisis needs to result in a hospitalization. Sometimes just talking with an understanding person is the best treatment mental health professionals can provide.

Mr. Hyde and nerve pain

I have been thinking the past few days. Yes that has been the smoke that has been circulating in the air in Boston. Apparently on Tuesday in the throws of another pain episode I was barely aware of, I sent my therapist a text. It said that I cannot go on living like this, with this level of pain. I have no recollection of sending it. I knew I sent it because it is my phone but it sounded more of Mr. Hyde. I have been trying to wrap my head around Hyde. He likes to come around at various times and sometimes I can “catch him” and other time he alludes me. But I am cracking down on when he is likely to occur. When I am deeply hopeless, suicidal, and in severe physical pain. All three must be present for Hyde to emerge. The sucky part about this is my safety because Hyde want to die. I wasn’t in danger of dying, as far as I know, Tuesday but I could have. I just wanted to sleep, and fairly succeeded on doing that. I also have to be in a sleep state to bring Hyde out. He is more likely to show himself while I am overtired. Like I am now.

I just had zaps go through my foot. Just imagine the shock you get when you place your tongue on a 9 volt battery, minus the metallic taste and you have the zaps I get in my feet. Sometimes they are stronger than a 9 volt. And they jolt me awake, like tonight. I was ready to turn in but my PTSD symptoms (hypervigilance and anxiety) have taken over. It is going to take another Ativan to calm down. Hyde doesn’t like Ativan too much. It makes me really sleepy so I do sleep. It doesn’t cause me to do other weird stuff like Neurontin does. I know that I am not having another attack of CES and that things will be ok but in my head, I am “excited” and fearful of when the next zap is going to come. Usually if I stretch out my legs, it makes it worse. So I have to sit with my legs up so they don’t get zapped. And there is nothing I can take for it. The zaps come and go as they please. They don’t last long enough for me to take anything, but boy do they cause havoc!!

I am listening to Luke Bryan. He has some really good songs that I love listening to over and over again. He has that southern voice that I just love. It can be a fast song or a slow one and I just swoon over his voice. It’s not relaxing like Mary Chapin, but it will do.

I really need to go to bed soon. It’s after midnight and I need to be up at six. I need to take a shower and get ready to take my sister’s car. I will need a large coffee to help my mood. The good thing is that I don’t need to rush because I don’t have to pick up my father till 830. I then have to take him for a blood draw for his Coumadin test. He doesn’t know it yet but he will.

Well, I think the zaps have settled down some. I hope I get some sleep tonight. Or tomorrow is going to suck…

Abyss of Darkness

Took yesterday off from blogging because I didn’t know what to write. Things have been so difficult lately that writing this blog has been rough. What has been my outlet is now a fullstop. I don’t even know if that last sentence makes sense. I just am very depressed.

It’s hard being depressed and not being able to express it like I have in the past. It’s killing me skipping days but my doc doesn’t want to put added pressure on me with this blog. I can’t even write in my journal. But I know that if I stop writing, things will get worse. I am always on the verge of suicide and not writing might just push me. So even though my pdoc wants me to cut down on my blogs, I just can’t. I might not write everyday like I used to, so don’t be shocked that the blogs are not consecutive like they once were. I know I have daily readers so I apologize but I got to take care of me in some way. If I feel like writing, I will and if I don’t, I just won’t. Hopefully there won’t be too many won’ts.

I visited my therapist today. There was no traffic, at all, so I got there two hours early. I shopped at the Whole Food Market but didn’t buy anything. What I wanted, they didn’t have. Or what they did have was too expensive. I really wanted carobs as I haven’t had them in years but all they had were carob chips and that wasn’t what I was looking for. I was really bummed. I then went to Starbucks and tried to write. Nothing worthwhile came to me.

My therapist finally got her signed copy of my book. She is overjoyed. We talked about the book some as I told her I was thinking of pulling the plug. At first she didn’t know what I was talking about, but then I explained that I was taking it off the market, and she freaked out. Well, not really, but she gave me the let’s talk about this glare. And I so didn’t want to talk about it. She still thinks that there is some merit in my book, just like this blog helps people. But I told her I just feel like I exposed myself too much and this is getting dicey. I still feel nervous while talking about the book, in detail, even while I was with her. The session just felt like it was going on forever. I know the book needs to go into the hands of suicide people and I don’t mean those that have attempted. Maybe clinicians. But I am not rich and can’t be shipping my book to all the psychologists in my area or around the US. It might just end up in the recycle bin. I am sending it to the AAS for review so I am hoping I get a good review and maybe it will sell a little better. If not, at least I have the AAS backing on my book which will mean more than anything. I am looking for validation and it’s killing me that I am not getting it. I guess that is why I am so depressed. My needs are frustrated. I still need to send my book to the consultant. But I am out of mailers. I will have to go to the post office tomorrow and do that.

We also talked about why I don’t want to “talk” in therapy. I told her I felt like I am wasting her time. I still feel like she could make better use of her time if she saw someone else. I guess I feel like I don’t deserve to be in treatment. It’s the same thing with my pdoc. I don’t know why she makes time for me. It’s not like things are going to change in a week and I am going to magically feel better. I am hopeless. I don’t think things are going to change. I am stuck in the abyss of darkness. Sure, things have gone a little better this week. I did get some validation on my book. And I know the word needs to be out there but am I the person to do it?

My therapist still thinks that it is quite an accomplishment to write a book and publish it. But I just feel bad because, like my blogs, they don’t have any messages other than my life sucks. I don’t leave any hopefulness in here nor the book. Call me the harshest critic, but the book is depressing. But then, when can you call a suicide attempt a happy event? You don’t. And not too many people know the extent of just how suicidal I have been over the years. My one regret is that I didn’t put the website for the CES support group in the book on the reference page. It completely slipped my mind, until now. It would seem silly to redo the whole process just to put in a website. I don’t think I can bear it again. Three or four times was enough.

how I manage being suicidal

It’s well past 2 in the morning. This may well be a Mr. Hyde blog as I am very tired but feel the need to write. Mr. Hyde likes to write things, very bad things and depressing things at this hour so this is a warning that this might be a suicidal blog.

I have been up the last few hours battling pain. My foot exploded around 11 pm (2300) and has now settled down some after putting on some gel and taking my pain meds. But then I got sick, I felt like I had to go throw up. So I laid down only it made it worse with reflux.

I wrote my psychiatrist a letter that I am hopelessly depressed and why bother with treatment of any kind as it is not helping me. I tried to get out of therapy with my therapist for today’s session and failed. I just don’t see the point. I am deeply depressed and if I could I would do something to end my life but I have no idea what I would do. Sure I have pills, but that might just make me sick and I hate to clean up vomit, if I survived. My luck, I probably will. I hate being in pain and can’t sleep. It drives me absolutely nuts.

A fellow blogger is battling her demons too. Her psych team wants to hospitalize her because she is suicidal. I suggested an alternative, the SSF to help deal with suicidal thoughts and to come up with a treatment plan. I told her to get the Managing suicidal risk book. It is a good book, if you are trying to manage suicidality. I don’t know what I did with my copy of the book. I know it is somewhere in my room or in my office. I can never find it when I need it. I have the SSF (suicide status forms) all over the place but not the actual book. And, no, because of copyright rules, I cannot post the forms as much as I would absolutely love to. There is one online, used, but helpful just to give you an idea of what they look like. I think I might ask my therapist to use it tomorrow. Or use Holden’s psychache scale. And again, as much as I would love to post it, I cannot because of copyright rules. I just am so hopeless. Everything is dark and gray, and I don’t mean the weather. I feel like I have no future, no purpose in life. Sure I published a book and that is a huge accomplishment. But why am I being “punished” with this depression?? What have I done that is so wrong? I hate my life.

My ex blocked me on Facebook today. I am actually glad because I was getting uncomfortable with the questions she was asking. She wanted to get back together. That is not going to happen. I guess me telling her I just wanted an online relationship pissed her off. Oh well. First time I have been blocked by someone. But this is kind of good because I don’t need her drama in my life. Yes, it bothers me but only because I thought this time we could just be friends and I have no idea what set her off and I will never know. Oh well.

Tonight was the first time all week that I took all my meds that I was supposed to take. I think that is why my stomach is bothering me. I usually have something to eat when I take them but tonight, I didn’t eat anything. I just am not hungry. And feeling sick to your stomach doesn’t make you want to eat anything. And oh joy, I think I may have a UTI. I have been leaking the past few days, more so than usual. Oh the joys of CES. That has me down too, because who likes to piss their pants? I am so tired of dealing with wet underwear. And having to take a shower every time I leak. It sucks because I hate showering. I had a good shower tonight. The water was nice and hot and it relaxed me. It was the first time in a while I felt that way. But I couldn’t stay too long because I knew my foot would act up and it did. Damn foot! Always ruins things. But I did a lot of stairs today and walking so it is my fault it flared up. I wish I could chop it off. Least with the ghost pain, it will be a real reason why it hurts. I don’t have a clear reason why my ankle/foot hurts. They think it is tendonitis. I think it is just nerve damage and over usage from fatigue. My foot gets tired and then it needs to rest but I don’t know it so I keep using it and then it flares up on me late at night. I then write blogs like this because I have nothing better to do and I can’t sleep anyways. I wish I was dead than deal with this pain every night. My heart is so heavy with heartache. I really don’t know why I keep going on. But tomorrow I will call my PCP and hopefully not talk to the stupid nurse about my UTI symptoms because other than leaking, I have no burning or pain. I don’t feel it because of nerve damage down there. I just have had bladder spasms. But those have subsided. But now my urine reeks so I know something is going on. Fucking CES always has to throw a wrench in the works. Can’t always be a simple case. I still will need to give a urine sample and I hope I will be able to. That is always the tricky part. I have to make sure I drink a lot before the appointment. Otherwise, I might not go when they give me the cup to pee. Oh the joys of retention! I no longer get the signal to my brain when I am full. Usually, I have to start leaking and then the signal goes to my brain that I am full. Fucking CES. Wrecks your life forever. And people don’t get it when you tell them. I was telling my cousin tonight the story about how I got CEs and the surgeries I went through. He still didn’t get it. But oh well. Not his life to live.
